Staff Directory- Contacts throughout the Ann's Choice Community
Most of the general numbers are answered by a live person during normal working hours. However, you may encounter answering machines and delayed responses when calling specific personnel since they may not be in their office or even on campus when you call.

Policy for Hiring External Private Duty Aides/Companions
Please be reminded of some – but not all - of the salient points of pet ownership responsibilities below.
- Pets must be on a leash at all times when outside the owner’s apartment
- Please take the most direct route to enter and exit all buildings when with your pet
- Pets are not allowed in the community buildings nor should they be walked in the courtyards. They are not allowed to sit on public furniture or to ride in any Erickson vehicle
- When on outside walks with your pet, please avoid high traffic areas, flower beds, and patios of other residents. Pet droppings should be picked up immediately and disposed of properly
- Pets are not allowed in the Fitness Center, restaurants, the hair salons, stores, offices, or banks
- Visiting pets are allowed on campus but are subject to the same conditions as resident pets
- Residents with a ground floor patio may tether their pet if:
- they remain outside with the pet
- the pet cannot go within two feet of a sidewalk or a neighbor’s patio
Please also remember that all resident pets must be duly registered through the Pet registration form that is kept on file by your Social Worker. Cats and dogs should be kept up to date with all required vaccinations and residents are required to maintain a vaccination record. Additionally, all dogs must be registered with the local county of residence.
For more detail on any of the above, or if you have questions, please contact your Social Worker.
Thank you for your anticipated compliance with these pet ownership responsibilities!

FILE OF LIFE
All of us who live here should have a "FILE OF LIFE" on their refrigerator, and it should be up to date. This is a form that we fill out and it contains items such as our medications, allergies, and contacts. This will be used in an emergency if medical people are called to our home to help us. EVERYONE SHOULD HAVE THIS. NO EXCUSES.
Blank File of Life forms can be found under the General Services heading on the PUBLICATIONS/FORMS tab above.
Suggestions for your Fridge.
The two files of life, a magnet listing the five stroke symptoms, trash and recycling days (along with the plastic recycling numbers of one to seven) and the hours of the Treasure Chest. Next to the kitchen phone are the emergency phone numbers. DO IT.
